Certificate of Completion
An original certificate is given to each student who has completed the 8-hour class. We maintain copies of all student certificates in their files in the classroom for a period of three years. There is a fee of $10 for a duplicate copy to be emailed, faxed or mailed to a student. Please click on the button below to order a duplicate certificate.

SCDMV Permit/License Application
Must be completed and used when applying for an SC Beginner's Permit or Driver's License.
SCDMV Consent for Minor
Must be completed and signed by a parent or legal guardian, and a copy of the signing parent's driver's license must be attached for us to take a student to the DMV for their picture.
SCDMV Application Supplement
Must be completed and used by all males aged 13-25 applying for a license or SC ID.
SCDMV PDLA form
Must be completed and used for all drivers under 17 who are testing for their restricted license. This form must be signed by a parent/guardian, your driving school instructor, AND an administrator from your high school.
SCDMV Certification of Conditions
Must be read and signed by student and parent/guardian for the student to be able to take the road test with a 123 Drive! Safety Officer.
